What IVIG Is and Why It's Used

IVIG is a concentrated preparation of IgG antibodies pooled from thousands of donors. In PANDAS, it is used to address the autoantibodies believed to cross-react with brain tissue — potentially by diluting them, providing anti-idiotype antibodies, and modulating immune activity. The landmark 1999 Swedo study showed IVIG was significantly more effective than placebo in severe PANDAS. Subsequent studies have been more mixed; practice patterns vary by institution.

Most insurers classify IVIG for PANDAS as "investigational" and deny initial requests routinely. Virginia enacted legislation (HB1641) requiring health insurers to cover IVIG when prescribed for autoimmune conditions in pediatric patients meeting specified clinical criteria. Families outside Virginia are navigating a patchwork of coverage policies. Prior authorization is nearly universal.

Prior authorization typically requires: letter of medical necessity from the prescribing specialist, documentation of the PANDAS diagnosis and clinical course, evidence of prior treatment attempts and response, and objective evidence of functional impairment. Many families report a brief symptom exacerbation in the days following infusion — this is well-documented and reflects immune activation.
